Management of Osteoporotic Subtrochanteric Femur Fracture

The subtrochanteric region of femur is defined as the 5-cm segment distal to the lesser trochanter. Fractures of this area can occur as a result of low-energy injury in the osteoporotic elderly patients and it can also occur after long-term intake of anti-resorptive agents. Cephalomedullary nailing is the gold standard of treatment for most subtrochanteric femur fractures. Adequate fracture reduction and proper nail entry point are critical. This article describes the pathoanatomy and surgical techniques essential in obtaining a successful outcome.
