In India, the health insurance coverage has increased from 5% in 2004–05 to 41% by 2019–21. Much of the increase in the insurance coverage has been due to social health insurance schemes such as the of Rashtriya Swasthya Bima Yojana (RSBY), the Senior Citizen Health Insurance Scheme (SCHIS), and the state health insurance schemes. Recently, the Ayushman Bharat Yojana (ABY), was launched in September 2018 with the objective to provide financial protection to the bottom 40 percent of India’s population. The ABY subsumed the previous schemes including RSBY and many state health insurance schemes. Though the insurance coverage in the country has increased over time, little is known about the insurance coverage of the elderly. This chapter examines the health insurance coverage among households with and without elderly members. A nationally representative sample of 636,999 households from the fifth round of the National Family and Health Survey, 2019–21, was used in the analysis. The coverage of insurance was examined for three subgroups of households: households with no elderly members, households with both elderly and nonelderly members, and households with only elderly members. The coverage of any health insurance and social health insurance by type of household, sociodemographic characteristics, and state were estimated. The coverage of health insurance was 40.5 percent among households with only elderly members, 41.9 percent among households with both elderly and nonelderly members, and 41 percent among households with no elderly members. There were large variations in health insurance coverage among states and socioeconomic groups. In most of the states, the health insurance coverage was the lowest among households with only elderly members compared to households without elderly members and households with both elderly and nonelderly members. The insurance coverage was significantly higher among households from the richest wealth quintile, those belonging to the scheduled castes, those headed by males, and those with elderly members.
