Segmental Anatomy of the Left Lower Lobe

Lower lobe, compared to the upper lobe, has more variations of segmental vessels, especially in vein. However, while the segmental vein is necessary to be identified for the identification of segmental artery in the segmentectomy of upper lobe, it is not necessary in the segmentectomy of lower lobe. In the segmentectomy of lower lobe, segmental arteries are identified and cut first, followed by cutting the segmental bronchus. Then, the segmental veins running toward the resecting segment are seen, which can be cut without identifying their names. Instead, in the segmentectomy of lower lobe, segmental arteries should be identified only from the MDCT findings and intraoperative findings, without identifying the segmental veins. The anatomical resection of lower lobe segment is dependent on the accurate identification of segmental arteries (Tables 66.1, 66.2, 66.3).
