Segmental Anatomy of the Right Lower Lobe

Lower lobe, compared to the upper lobe, has more variations of segmental vessels, especially in segmental vein. However, while the segmental vein is not necessary to be identified in the segmentectomy of the lower lobe, whereas it is necessary in the segmentectomy of the upper lobe. In the segmentectomy of the lower lobe, segmental arteries are identified cut first, followed by cutting the segmental bronchus. Next, segmental veins running toward the resecting segment are cut, which does not need the identifying each name. Instead, in the segmentectomy of lower lobe, segmental arteries should be identified from the MDCT findings and intraoperative findings, without referring to the route of segmental vein. The anatomical resection of lower lobe segment is dependent on the identifying segmental arteries correctly (Tables 29.1, 29.2, and 29.3).
