Proximal hypospadias poses challenges for surgical repair due to various anatomical factors, leading to high complication rates and the need for reoperations. Current approaches lack robust data, complicating treatment decisions. Surgical techniques can be broadly categorized into urethral plate preservation and transection procedures. Utilizing vascularized preputial flaps has been a key advancement in severe hypospadias repair, although concerns remain about dorsal penile skin ischemia post-surgery. Innovations such as the double-face preputial island flap procedure have improved single-stage treatment options. This summary highlights ongoing advancements in hypospadias repair, including our standardized approach to the double-face preputial island flap procedure.
