MRI breast is the most sensitive investigation for detection of breast cancer among current radiological imaging modalities. While standard protocol is very useful in clinical application, recent advances in technologies and software have been continuously improving the diagnostic accuracy and its use in the management of breast cancer. Indications of the breast MRI extend from diagnostic indications to preoperative planning, treatment monitoring as well as screening. American cancer society recommends annual MRI screening in addition to mammography for the women with lifetime risk of breast cancer is more than 20% (Saslow D, Boetes C, Burke W, Harms S, Leach MO, Lehman CD, Morris E, Pisano E, Schnall M, Sener S, Smith RA, Warner E, Yaffe M, Andrews KS, Russell CA, CA Cancer J Clin. 57(2):75–89, 2007). Proper technique, postprocessing and interpretation are very important in the quality reporting of the breast MRI. This chapter focuses on the standard and advanced techniques of breast MRI and its clinical application.
