The infectious disease (COVID-19) of the new coronavirus (SARS-CoV-2) begins when SARS-CoV-2 binds to the virus receptor in the upper respiratory tract. The virus receptor of SARS-CoV-2 is angiotensin-converting enzyme 2 (ACE2). ACE2 is widely expressed on the endothelial cell membrane of blood vessels throughout the body, as well as on the airway epithelium and type II epithelium of the alveoli. Therefore, COVID-19 is a complex disease state that should be described as a systemic multi-organ inflammatory disease accompanied by vasculitis/coagulation disorders/cytokine storms due to vascular endothelial injury, rather than a mere respiratory disease. In fact, in most severe cases of COVID-19, the focus has been on calming uncontrollable inflammatory responses. However, exposure to SARS-CoV-2 does not necessarily lead to the onset and severity of the disease. This is because an appropriate natural immune response suppresses the onset or leads to recovery even if the disease does occur.
