Venous Thromboembolic Disease
摘要
Venous thromboembolism (VTE) is a major global health concern associated with significant morbidity and mortality. It encompasses deep vein thrombosis (DVT) and pulmonary embolism (PE), arising from the classic triad of venous stasis, endothelial injury, and hypercoagulability, with both provoked and unprovoked risk factors. Clinical presentations vary widely, from asymptomatic cases to life-threatening complications such as massive PE. DVT often presents with limb swelling and pain, while PE may manifest as dyspnea, chest pain, or hemodynamic instability. Diagnosis is guided by clinical prediction tools such as the Wells score, D-dimer testing, and confirmatory imaging including venous ultrasonography and CT pulmonary angiography. Management primarily involves anticoagulation with agents such as warfarin or direct oral anticoagulants (DOACs), with thrombolysis reserved for severe or unstable cases. Prevention strategies, particularly thromboprophylaxis in high-risk populations, are essential to reduce VTE incidence and recurrence.
