We present a case study of a 56-year-old female maintenance hemodialysis patient with a complex medical history, including kidney transplantation, decompensation of liver cirrhosis and positive erythrocyte irregular antibody. The patient was admitted to the hospital due to persistent fatigue, diminished appetite, and intermittent melena with a hemoglobin level of 55 g/L. Conventional management strategies for her anemia proved ineffective. Following a multidisciplinary approach and thorough clinical evaluation, Roxadustat 100mg thrice a week was administered for treatment for anemia. This intervention resulted in a significant increase in hemoglobin levels (80-100 g/L) and an improvement in her clinical symptoms. This case illustrates the potential efficacy of Roxadustat as a therapeutic option for patients with renal anemia, particularly in complex clinical scenarios characterized by iron deficiency, hypersplenism, gastrointestinal bleeding, and erythropoietin resistance. The findings from this case contribute valuable insights and references for the management of anemia in similar patients in the future.
