Role of Therapeutic Potential, Contemporary Management, Unmet needs of SGLT2 Inhibitors in Congestive Heart Failure

Congestive heart failure (CHF) is a critical public health issue impacting millions worldwide. Early diagnosis and tailored treatment strategies are vital to improve outcomes. Sodium-glucose cotransporter-2 inhibitors (SGLT2i), initially developed for Type 2 diabetes management, have shown promise in treating heart failure (HF) in both diabetic and non-diabetic individuals. Large studies like DAPA-HF and EMPEROR-REDUCED demonstrate reduced cardiovascular mortality and heart failure hospitalizations among patients with reduced ejection fraction, independent of diabetes status. Furthermore, the SOLOIST-WHF and EMPEROR-PRESERVED trials reveal benefits of SGLT2i in hard-to-treat HF populations with preserved ejection fraction, showing significant reductions in HF hospitalizations and improved quality of life. The DELIVER study confirms SGLT2i’s benefits for HF with mid-range and preserved ejection fraction. These findings underscore the broad applicability of SGLT2i across diverse HF subtypes, advocating their integration into standard CHF management.
