摘要

Operating experience feedback is an important part of human factors engineering in nuclear power field. High quality operating experience feedback analysis can deeply explore common problems, key problems and forward-looking problems, and provide effective reference for subsequent hidden danger troubleshooting and design improvement, so as to prevent accidents and improve the safety level of nuclear power plants. Root cause analysis is an important process in the process of operating experience feedback organization and management, and it is a process in which people further explore and find the root cause of events based on the observable direct causes. At present, “5Why” analysis, fishbone diagram, fault tree analysis and other tools are often used in the process of event root cause analysis. In practice, it is found that the current analysis method has some limitations in the application of some event analysis, insufficient objectivity and difficulty in determining the deepest level of event failure factors. On this basis, this paper uses Delphi analysis and analytic hierarchy process to develop a new root cause analysis workflow. At the same time, taking a certain operation event as an example, it elaborates how to quantitatively calculate the relative weight of each influencing factor that affects the occurrence of the event through the workflow. And through analysis, we can finally get the process of the root cause of the event.
