Dysthyroid optic neuropathy (DON) is the optic nerve damage resulting from the compression of the optic nerve in thyroid orbitopathy. This compression could be due to pressure exerted by the enlarged extraocular muscles (in 95% of cases) or increased fat causing stretching of the optic nerve (in 5% of cases) in the tight bony orbit cavity [1]. DON is a serious complication of thyroid eye disease seen in 6–7% of the TED cases [2, 3]. It is an ocular emergency, but the vision loss in this condition can be reversed with timely treatment. It is therefore a definitive indication for surgical decompression.
