Anastomotic Urethroplasty in Pelvic Fracture Urethral Injuries (PFUIs)

Anastomotic urethroplasty is the gold standard surgical technique for treating pelvic fracture urethral injury (PFUI), which has an incidence of 10% of pelvic fracture cases [1, 2]. This procedure involves excision of scar tissue to reconnect the urethral ends and boasts a success rate of 80–97% [3]. The majority of PFUIs are classified as simple and usually are successful in the first anastomotic surgery [3]. While PFUI repair is a complex surgery, it can yield a high success rate, especially when performed by experienced reconstructive urologists. Poor outcomes are often associated with the length of the stricture, the proximity to the bladder neck, and the presence of a concomitant recto-urethral fistula. It is advisable to wait 3–6 months after the injury before proceeding with urethroplasty in PFUI [4].
