The patient, a woman in her 30s, has not menstruated for 10 weeks and arrived at our hospital with a positive pregnancy test and sudden lower abdominal pain. An ultrasound scan did not show a fetus in the uterus but revealed a retained hematoma in the left fallopian tube and an echogenic-free space in the Douglas-fossa. If imaging conclusively indicated a tubal pregnancy, we resolved to perform emergency surgery. This scenario presents an opportunity to discuss how to diagnose conditions using imaging.
