Posterior Stabilized Total Knee Arthroplasty (PS-TKA) (Gap-Balancing Technique)

PS-TKA uses a post–cam mechanism that compensates for posterior cruciate ligament (PCL) function to improve the range of motion (ROM) and anteroposterior instability of total condylar TKA, an anterior cruciate ligament (ACL)/PCL resection-type TKA. PS-TKA has good long-term durability even when osteotomy is performed to the thickness of the femoral implant (measured resection technique). However, the measured resection technique often results in a large flexion gap because the PCL is resected [1–4] A large flexion gap induces midflexion laxity [5, 6] and instability. A surgical technique that adjusts the osteotomy of the posterior condyle (gap balancing technique) before that of the posterior condyle [7] can reduce laxity in the midflexion position.
