Migraine, a formidable neurovascular challenge, maintains its impactful presence, influencing the lives of more than one billion individuals globally. Pulsating headaches are a hallmark feature of migraine and are often accompanied symptoms including sensory disturbances, nausea, vomiting, pain, and phonophobia. These debilitating headaches are affected by both genetic and environmental factors. The associated disabilities have a range of negative impact not solely on those directly affected but also on their families, co-workers, employers, and the broader societal framework. Understanding the profound effects of migraine prompts a deeper understanding into its mechanism. The mechanism involves intricate interactions within the trigeminovascular system, activating peripheral and central pathways. The disruption of neurotransmitter levels sets in motion a cascade of events, involving cortical spreading depression and modulation of pain pathways. Cortical spreading depression is viewed as a crucial factor in migraine aura and contributes to the visual disturbances and sensory alterations characteristics of migraine auras, Furthermore, central modulations involve alterations in the processing of pain signals within the central nervous system, influencing pain perception and sensitivity. On the peripheral level, there is an influence on the relay of signals from the periphery to the centre. Understanding these processes is necessary for advancing therapeutic strategies and refining strategies to alleviate the burden of migraine.
