摘要

The present study investigated risk assessment in the hydraulic cylinder assembly industry. This is achieved by identifying the leakage types, causes, etc. The Failure Mode and Effect Analysis (FMEA) was implemented to determine the root cause by computing Severity (S), Occurrence (O), Detection (D), and Risk Priority Number (RPN) for each failure mode. The RPN value highlights the risk of failure in current risk assessment methods. However, RPN has certain shortcomings. Hence, the FMEA method is modified with the multi-decision criteria method decision-making trial and evaluation laboratory (DEMATEL). By integrating DEMATEL with the FMEA method, clear identification of the most influencing causes (root causes) and their most controlled effect are identified. After identifying the root causes, specific measures for eliminating these root causes are implemented. It was found that the proposed integrated method provided valuable information for the decision-makers in identifying the critical problems and prioritization of the solutions.
