The Problem-Oriented Medical Record (POMR) for Outpatients

Outpatients are individuals who receive medical treatment in the outpatient department or clinic without being admitted to a hospital. They visit the clinic regularly or irregularly to seek treatment for their issues. At each visit, the clinician should complete the patient’s corresponding medical record. Although the original Problem-Oriented Medical Record (POMR) was not specifically designed for outpatient care, the same principles can be applied to both inpatients and outpatients. I propose the following format for the outpatient’s POMR.
