Yoga, Pilates, and Alternative Therapies: Evidence-Based Review

Yoga, Pilates, and alternative therapies (like manual therapy, acupuncture, acupressure, Tai Chi and Qigong, massage therapy, Kinesio taping, cupping) emphasize active, patient-centered approaches that build strength, improve flexibility, and incorporate mental well-being, moving beyond simple symptom relief to address the underlying physical and psychological aspects of chronic pain. The integration of these therapies within a personalized rehabilitation plan can significantly enhance outcomes and quality of life for those living with LBP. Yoga provides a safe, cost-effective, and holistic intervention for long-term LBP management. Yoga supports the biopsychosocial model by addressing physical impairments, psychological factors, and functional limitations simultaneously. Acute LBP cases require low-intensity, pain-free yoga focusing on relaxation and autonomic regulation to prevent symptom exacerbation. Chronic LBP cases benefits from progressive loading, longer holds, and postural endurance training to enhance spinal stability and neuromuscular control. Pilates focus on core stabilization, postural alignment, neuromuscular control, and movement precision. Manual therapy includes joint mobilization, manipulation, soft tissue techniques, and muscle energy techniques. Acupuncture is a traditional Chinese medicine (TCM) technique to modulate physiological functions and relieve pain. Tai Chi and Qigong combine gentle aerobic exercise, balance training, and mindfulness and help in reducing pain and improving proprioception. Massage therapy is a manipulation of soft tissues to relieve muscle tension, improve circulation, and reduce pain. Acupressure is a simple, noninvasive therapeutic technique. Other alternative techniques are Kinesio taping and cupping therapy which are also used in treating low back pain. Overall, yoga, Pilates, and alternative therapies represent valuable adjuncts in evidence-based practice, offering holistic, patient-centered approaches that support long-term self-management and functional recovery.
