Modern Concepts in Spine Physiotherapy and Core Stabilization

Traditional physiotherapy is a fundamental approach in the back pain management and provides short-term improvements in pain and function. However, its overall efficacy rate is moderate, and outcomes are often limited due to poor adherence, insufficient psychosocial integration, and variability in clinical practice. These challenges can be addressed through individualized care, patient education, and incorporation of biopsychosocial principles which enhances the effectiveness of physiotherapy interventions for back pain. Modern physiotherapy is an advance approach for the management of low back pain by integrating physical, psychological, and social dimensions of care. Modern physiotherapy also emphasizes on active rehabilitation, patient education, and self-management which leads to superior long-term outcomes compared to traditional physiotherapy. Continued training, research, and system-level support are essential for its effective implementation in clinical practice. Core stabilization is an essential component of modern low back pain management. By focusing deep trunk muscles and improving neuromuscular control, core stabilization enhances spinal stability, reduces excessive mechanical stress on spinal structures, and improves functional capacity. Evidence-based practice indicates that core stabilization is more effective in chronic and recurrent low back pain, leading to meaningful reductions in pain, disability, and recurrence when compared with general exercise alone. When integrated with patient education, graded functional training, and a biopsychosocial approach, core stabilization supports long-term self-management and sustainable outcomes. Therefore, core stabilization should be considered a key evidence-based strategy within comprehensive physiotherapy management for lower back pain.
