Advanced MRI Techniques in Lumbar Spine Pathology

Degenerative disorders of the lumbar spine are a leading cause of chronic low back pain, and conventional magnetic resonance imaging (MRI), while essential, often shows limited correlation with clinical symptoms due to its primarily qualitative nature. This clinicoradiological mismatch has led to the development of advanced MRI techniques that provide quantitative and functional assessment of spinal tissues. Techniques such as T2 mapping, diffusion-weighted imaging, diffusion tensor imaging, and advanced evaluation of Modic changes offer deeper insights into disc biochemistry, tissue hydration, neural microstructure, and vertebral endplate biology. T2 mapping enables early and objective detection of disc degeneration by quantifying water and proteoglycan content. Diffusion-based methods aid in assessing disc integrity, differentiating between degenerative and infectious conditions, and evaluating nerve root compromise. Advanced characterization of Modic changes helps identify biologically active pain generators with prognostic and therapeutic relevance. A multiparametric MRI approach thus bridges the gap between imaging and clinical decision-making in degenerative lumbar spine disease.
