Microbiome-Based Formulations and Products and Their Therapeutic Implications

The profound impact of microbial communities on human health is revolutionizing diagnostics and therapeutics. In the face of escalating microbial resistance, the traditional 1950s approach of using antimicrobial drugs is yielding a new understanding of the crucial role played by diverse microbial ecosystems in maintaining health. Microbiome medicines can be categorized by biological concepts, therapeutic domains, and “bug status.” “Bug status” categorization is instrumental in positioning microbiome therapeutic endeavors. It encompasses three key categories: “drugs from bugs,” where medicines are derived from microbial sources; “drugs for bugs”, interventions that support microbial communities with specific nutrients; and “bugs as drugs,” where live microorganisms, termed Live Biotherapeutic Products (LBPs), are administered for disease prevention or treatment. Fecal microbiota transplantation (FMT) is resurging as a treatment for Clostridium difficile infections (CDI) and inflammatory bowel disease (IBD). However, advancing these approaches necessitates further research into the roles of microbe–host interactions and host-associated microbial ecosystems. It also requires improvements in engineering techniques to modify bacterial hosts, develop disease-specific sensors for targeted therapeutic production, and ensure reliable in vivo functionality. These advancements hold the key to practical applications in microbiome therapeutics, offering hope for more effective treatments in the future.
