OCT in PDEK and DMEK

Graft adherence is considered as the single important factor for better functional outcome after successful endothelial keratoplasty [1, 2]. Graft detachment has been described as a common complication after endothelial keratoplasty techniques like DMEK and DSAEK [1–3]. Though thinner grafts are more susceptible to incomplete graft adhesion after primary positioning; early and possibly better visual recovery may be possible only with thin grafts [4]. Clinical slit lamp examination has always been the gold standard method of evaluation of graft position postoperatively. However in eyes with corneal edema or corneal haze, the position of graft and delineation of graft is difficult. Optical coherence tomography technique aids in easy visualization and proper documentation of posterior corneal layers [5–7]. Spectral domain optical coherence tomography (OCT) has been used in corneal evaluation for Descemet’s membrane evaluation and has been used widely for follow-up and prognosis assessment [7].
