Complications in DMEK: Prevention and Management

Descemet membrane endothelial keratoplasty (DMEK) is considered the best surgical treatment for endothelial dystrophy and pseudophakic bullous keratopathy (PBK). The diseased host corneal endothelium and Descemet membrane is replaced with corresponding healthy donor tissue through a small incision. DMEK provides better visual and refractive outcomes, faster recovery, and lower risk of immunologic rejection than penetrating keratoplasty (PK) or earlier iterations of endothelial keratoplasty (EK). However, as with any other surgical procedure, complications may occur. Careful case selection and good training can minimize DMEK complications.
