Effective strengthening relies on using an appropriate amount of resistance, and determining this through repetition numbers is more convenient than simply relying on the percentage of a one-repetition maximum in people with lower motor neuron lesion. The recovery of strength in patients with upper motor neuron lesion is achieved through several mechanisms, including diaschisis, disinhibition, and plastic changes in synapses. Among these mechanisms, changes in synapses are the most important for regaining muscle strength induced by training. Achieving changes in synapses requires numerous repetitions. When selecting resistance for strengthening in people with upper motor neuron lesions, it is imperative to ensure that it does not lead to incorrect movements, such as unnecessary associated movements or uncontrolled contractions.
