Safety professionals make use of several methods to investigate the root cause aiming for arriving at corrective steps and prevention of similar nature of accidents. Most regulatory review committees employ this root cause analysis techniques in the detailed investigations of post accidents and incidents and the remedial measures to prevent recurrences or minimize the frequency of occurrences. This paper discusses on the root cause analysis technique applied for investigation of a few Industrial accidents/incidents in a Research and Development Organisation.
