Posterior Pelvic Tilt Cases (Spinal and Pelvic Alignment)

Osteoarthritis of the hip results in severe degenerative changes to the spinal column, which often causes disruption to the sagittal alignment requiring corrective surgery. The risk of dislocation after total hip arthroplasty (THA) is higher in patients undergoing both THA and spinal fusion. A large survey indicated that performing spinal fusion followed by THA results in a higher dislocation rate than performing THA before spinal fusion [1]. Increasing the interval between THA and spinal fusions decreases the dislocation rate [1]. Therefore, if the situation permits, the risk of dislocation may be reduced by performing THA first followed by spinal fusion after an extended period. There is an increase in the risk of hip joint degeneration and the need for THA after long fusion of the spine [2]. Thus, the number of complicated THA cases after spinal fusion is likely to increase in the future based on the recent increase in the number of long fusion cases.
