The growing field of cancer research has revealed the complex interaction between epigenetic modifications and the development of drug resistance in response to chemotherapy. A heritable alteration in gene expression that is unrelated to variations in the sequence of nucleotides is known as epigenetic regulation. Histone modification, noncoding RNA regulation, and DNA methylation have all been thoroughly investigated as common epigenetic regulatory mechanisms. The chapter explores the molecular intricacies of DNA methylation and histone modifications, elucidating their influence on the establishment and maintenance of drug resistance in various cancer types. Epigenetic alterations not only serve as pathways for the evolution of drug-resistant phenotypes but also emerge as potential therapeutic targets for overcoming treatment resistance.
