Chronic diseases are long-lasting health conditions that often require ongoing medical care and treatment, substantially impacting individuals’ ability to perform daily activities normally. These conditions are the leading causes of mortality and morbidity worldwide placing significant burdens on individuals, families, and healthcare systems. Since people with chronic diseases require lifelong care, self-management becomes an essential survival strategy. Through effective self-management, individuals can take an active role in managing their condition, adhering to treatments, and adjusting their lifestyle activities to enhance their quality of life. In recent years, codesign-based interventions have emerged as a promising approach to improving self-management behaviors among individuals with chronic diseases. This collaborative method actively involves individual patients, healthcare providers, and other stakeholders in the design process that fosters the development of tailored solutions that resonate with end users. By ensuring interventions are both practical and relevant, codesign effectively addresses the real-world challenges faced by individuals and healthcare professionals. This chapter explores how codesign-based interventions can bridge gaps in healthcare and then introduces an example of codesign intervention created to enhance self-management behaviors among people with chronic kidney diseases in Vietnam.
