Stroke is a vascular accident in brain, manifests as a sudden disruption of blood vessels. Brain injury leads to the insufficient oxygen supply to the body parts causing paresis in an individual. Eventually, the patient losses muscle strength, balance, grip and cognitive thinking as well as memorizing capabilities. Rehabilitative training empowers stroke survivors to regain function and rebuild their lives. Prior to this stage of training, a physiotherapist needs to identify the level of severity of stroke the patient is affected with. Analyzing the stroke level is done by asking the patient to participate in gold standard assessment protocols focused on both motor and cognitive functions. The study investigates various motor and cognitive functionality assessment questionnaires and protocols used worldwide and their efficiency in identifying the level of severity of stroke and their capability to perform daily activities. Few globally standard assessment tools were taken as a point of consideration in this preliminary study and by implementing these protocols in clinical trial sessions. Ten stroke survivors within the age group of 40–80 years were participated in this pilot study. From these obtained scores and predicted stroke severity, therapy module allocation for rehabilitation is done by physicians. In future, design and development of patient-centric Virtual Reality (VR) based rehabilitation modules needed to done meticulously by referring these assessment scores as foundational benchmarks.
