This chapter explores the role of genetic predispositions, particularly the HLA-B27 gene, in the development of ankylosing spondylitis (AS). It aims to equip clinicians with the ability to recognize and differentiate the typical signs and symptoms of AS from other forms of arthritis by carefully assessing clinical presentations. The radiographic assessment of sacroiliitis and spinal changes, such as syndesmophytes and bamboo spine, will be highlighted. Additionally, this chapter reviews key diagnostic criteria, including the modified New York classification and the Assessment of Spondyloarthritis International Society (ASAS) criteria for axial spondyloarthritis. Finally, an overview of treatment options, lifestyle modifications, exercise regimens, and strategies for managing chronic pain and stiffness will be discussed to guide patient care.
