Continuous and safe process operation, along with higher profits, credibility and reputation are fuel for success in any process industry. Human factor remains pivotal in achieving these objectives. Human error has become a necessary concept within safety frameworks. Without thoroughly exploring the recent literature on human errors in the process industry, it is unwise to develop strategies for improving human performance or reduce human error in the future. Therefore, this study conducts a literature review from 2013 to 2023 by examining how concept of human error has advanced, its interpretation, and demographic factors like geography and orientation by discipline are assessed to provide a complete holistic view. Furthermore, the definition of human error and methods used across this industry are also investigated in the literature. Findings from the review showed a varied collection of methodologies for investigating human error, including systematic and cognitive approaches. Interpretations of human error vary, with some studies emphasizing individual mistakes while others highlight systemic issues. Additionally, demographic factors such as geography and disciplinary orientation influence the perception and characterization of human error within process industries. The insights from this work lay a foundation for future research and the development of specific methods to improve human performance, encouraging higher safety in process industry and other socio-technical systems (e.g., maritime, healthcare and chemical).
