While the early mortality rate after combined heart and lung transplantation (HLTx) is reported to be as high as 25%, three cases of HLTx have been performed at Osaka University with satisfactory long survival of 8.5–16 years. Although UNOS data indicated the risk of surgery is high in low-volume centers, HLTx could be safely performed in selected patients based on a surgical strategy that is meticulously discussed in a multidisciplinary transplant team. This review illustrates the details of three cases from the standpoint of surgical strategy in the very complex transplant procedure.
