Surgical paddle lead for spinal cord stimulation (SCS) is a cornerstone therapy for treating failed back surgery syndrome (FBSS), offering the advantages of lower impedance and reduced battery usage due to higher energy efficiency. However, the more invasive nature of this surgical procedure can lead to complications This chapter introduces a novel spinal cord stimulation technique using the unilateral biportal endoscopic (UBE) approach, illustrated through intraoperative photographs and high-definition surgical videos. The absence of lead migration after permanent paddle lead implantation confirms its stability and suggests it as a promising alternative for treating failed back surgery syndrome (FBSS).
