In recent years, the identification and management of concussion in cricket has become far more important due to emerging research linking concussions and repetitive head trauma in sport to long-term neurocognitive issues. Accurate and prompt diagnosis is essential to mitigate risks such as prolonged recovery, compromised cricket performance and, possibly, long-term health effects. The introduction of concussion substitutes by the ICC and heightened awareness across other sports has highlighted the need for optimal concussion management in cricket. Incidence rates indicate higher concussion occurrences in shorter formats, with head impacts in batters facing fast bowlers posing the greatest risk. Concussions in training are common. Fielders are vulnerable when attempting catches, sometimes through a mechanism of whiplash. Gold-standard concussion identification involves video analysis for immediate signs and on-field assessments using modified Maddocks questions. The SCAT6 tool is the current standard for off-field evaluations. Concussions in cricket can present with delayed symptoms only, so repeat assessments are useful for capturing these delayed symptoms. Cricket-specific return-to-play protocols involve returning to ground fielding and throwing drills before progressing to full catching drills, facing throwdowns and spin, followed by progressing to facing pace bowling. Difficult cases necessitate specialised rehabilitation, particularly for vestibular or neck-related symptoms. Athletes with persistent symptoms or multiple concussions should consult practitioners with sufficient expertise in managing complex concussion cases.
