Lumbar Stress Fractures Prevention

Lumbar bone stress injuries (LBSI) create the greatest injury burden within the game of cricket, often requiring between 6 and 9 months of rehabilitation and time lost from the game. Recurrence rates in the professional game are high; therefore, identifying and implementing a strategy to reduce the risk of these injuries is of the utmost importance. Injury prevention is complex, and a methodical approach to identifying the risk factors associated with LBSI is essential before taking appropriate measures to mitigate modifiable risks. There have been multiple changes in technical models over the last few decades in the hope that these may reduce injury risk but fail to account for the large individual anatomical variations and techniques between players. Bowling guidelines were introduced to limit high bowling loads, but it is not clear whether these have affected injury rates, especially in community settings where reporting of low back pain in junior players is less robust. Despite large-scale implementation of exercise-based injury prevention programmes (IPPs) in other sports, cricket has yet to adopt a large-scale, evidence-based IPP that addresses the known risk factors associated with LBSI. Perhaps the most exciting area that is emerging is the role of education in injury prevention and how pivotal that might prove to be in the early identification of LBSI and in teaching younger players how to monitor and adapt training loads.
