This chapter presents adaptive planning for primary septorhinoplasty using soft tissue cephalometric analysis (STCA). By analyzing 2D photographs just before surgery, surgeons adjust magnification, pitch, roll, and yaw to tailor surgical plans. Cases include deviated noses and combined reduction-augmentation, emphasizing autogenous tissue preparation, septal L-strut extension grafts, and long-term outcomes to optimize function and aesthetics.
