Prolonged pre-surgical analysis and goal-setting can detract from the primary focus of surgery preparation. This chapter introduces soft tissue cephalometric analysis (STCA) as an efficient method for planning primary septorhinoplasty using standardized 2D photos. By adjusting magnification, pitch, and roll, surgeons define key landmarks—sellion, subnasale, and tip—for tailored surgical strategies. STCA addresses challenges unique to Asian rhinoplasty, including low radix positioning and nasal asymmetries, ensuring personalized and precise outcomes. The method integrates patient consultation, minimizes reliance on 3D simulations, and emphasizes practical preoperative analysis for optimal surgical planning and long-term results.
