A 32-year-old female with no significant medical history presented with fever, cough, sore throat, and shortness of breath for 3 days. She reported worsening symptoms, including fatigue and muscle aches and had a respiratory rate of 36/min. Oxygen saturation (SpO2) was 80% on room air. ABG showed moderate hypoxemia (PaO2 45 mmHg). Chest X-ray showed bilateral infiltrates and PCR testing confirmed H1N1 influenza infection. Given the worsening respiratory status, she is started on a nonrebreathing mask with oxygen flow at 15 L/min. Her saturation (SpO2) was still 86%, with a respiratory rate of 30/min.
