A 58-year-old obese male having a body mass index of 34 with diabetes mellitus and hypertension and diagnosed with acute pancreatitis was transferred from the ward to the ICU because of his altered mental status and respiratory distress for 4 h. At the time of presentation to the ICU, his GCS was ICU 13/15, he was lethargic, arousable, and had a respiratory rate of 34 breaths per minute with an oxygen saturation (SpO2) of 89% on oxygen given through a rebreathing face mask at 12 L/min. His heart rate was 120/min with blood pressure recorded at 110/70 mmHg. His chest X-ray is shown in Fig. 1.1.
