Anesthetic Management of Airway Bleeding During Transcatheter Occlusion of Major Aortopulmonary Collaterals in a Child with Fontan Circulation

A 13-year-old girl was scheduled for transcatheter occlusion of major aortopulmonary collaterals under general anesthesia due to recurrent hemoptysis. She underwent a Fontan procedure 3 years ago. Massive airway bleeding occurred during cardiac catheterization intervention. After pure oxygen ventilation, high positive end-expiratory pressure ventilation, epinephrine spraying via endotracheal tube, and thorough cleaning of the airway blood, the airway bleeding was well-controlled. The transcatheter occlusion of major aortopulmonary collaterals was successful. The patient was returned to the cardiac intensive care unit with endotracheal intubation. The anesthetic management of such patients is to quickly and effectively control airway bleeding, timely assess the cardiac function and hemodynamic status, and follow the anesthetic management principles of patients with single ventricle physiology.
