A 7-month-old female infant was diagnosed with Tetralogy of Fallot (TOF) after birth. She was scheduled for a preoperative enhanced cardiac CT scan. She experienced two hypoxic episodes, and her pulse oxygen saturation (SpO2) was 78% with air breathing. CT scan was successfully finished after intranasal administration of 20 μg dexmedetomidine and 10 mg ketamine. The key point of sedative management in children with TOF is to avoid or prevent the occurrence of hypoxic episodes.
