A 9-year-old boy with a history of complex congenital heart disease, diagnosed at birth, experienced frequent atrial flutter following multiple surgeries, including bidirectional cavopulmonary shunt, total anomalous pulmonary venous return correction, and common atrioventricular valvuloplasty. Recently, the episodes of atrial flutter increased significantly, and the patient was unresponsive to medication. The boy also reported fatigue and a notable decline in exercise tolerance. He was scheduled for radiofrequency ablation under general anesthesia. It is very important to maintain sinus rhythm, control the ventricular rate effectively, and avoid the occurrence of tachycardia during anesthetic management.
