A 9-year-old boy with unusual facies, wide-slanting forehead, hypertelorism, ptosis, and mandibular retrusion was diagnosed with Noonan syndrome through a genetic test. Echocardiography showed left ventricular hypertrophy. The activity of the coagulation factor was reduced. He was brought to our hospital to see a doctor due to snoring and mouth breathing during night sleep. A polysomnogram indicated severe obstructive sleep apnea–hypopnea syndrome (OSAHS). He was scheduled for an elective tonsillectomy and adenoidectomy under general anesthesia. The key point of anesthetic management for this patient includes the management of difficult airway, hemodynamics, and coagulation abnormalities.
