A 16-year-old girl with a history of Kawasaki disease experienced chest tightness and pain without obvious cause recently, and the symptoms became worse after physical exertion. Preoperative examinations showed coronary artery stenosis with a giant coronary artery thrombus. She underwent giant thrombus removal from the left coronary artery and anastomosis of the left internal mammary artery with the obtuse marginal branch under cardiopulmonary bypass. The anesthetic management should balance myocardial oxygen supply and demand and maintain myocardial contractibility during the perioperative period.
