A 2-year-old girl was diagnosed with the right aortic arch and subclavian artery pseudoaneurysm (Kommerell’s diverticulum) during the fetal period. She had been followed up since birth and was scheduled for vascular ring correction surgery. In such cases, Kommerell’s diverticulum may cause compression of the trachea and esophagus. Furthermore, cerebral hypoxia may happen due to the cross-clamping of the left common carotid artery and subclavian artery to help resection of the diverticulum and anastomosing of the subclavian artery to the carotid artery. Therefore, anesthetic management should focus on assessing and managing airway stenosis and cerebral oxygen monitoring during the perioperative period.
