Anesthetic Management of Arterial Switch Operation in a Child with Taussig–Bing Anomaly

A 4-month-old male infant was diagnosed with transposition of the great arteries and double outlet right ventricle (Taussig–Bing anomaly) due to cyanosis of the lips and fingertips. He was scheduled to undergo an arterial switch operation (ASO) under general anesthesia. After the aortic cross-clamp was released, the heart quickly resumed the sinus rhythm without defibrillation, but the electrocardiogram showed severe ST segment depression. The ST segment gradually returned to normal after treatment with dopamine, epinephrine, norepinephrine, and cardiopulmonary bypass support. The goals of perioperative management for such cases are to optimize cardiac output and balance myocardial oxygen supply and demand.
