Evidence suggests that social media provides valuable opportunities for disseminating health messages to diverse audiences, potentially influencing public health perceptions and behaviors. The expression of emotions on social media allows health promotion practitioners to assess public attitudes toward health promotion interventions and related behaviors. This study analyzed Twitter data to investigate trends in user engagement and sentiment towards “American Heart Month”, a prominent campaign promoting heart-health behaviors. 73,288 tweets were collected from January 2019 to March 2023 searching “#AmericanHeartMonth,” and 141,793 tweets were collected from January 2021 to March 2023 searching the campaign’s most popular hashtags. A lexicon-based sentiment analysis, a natural language processing (NLP) technique, was applied to categorize tweets into positive, negative, and neutral sentiments. The analysis revealed a significant positive correlation between user engagement (p-value = 0.008) and the dissemination of heart health-related information. The average positive sentiment ratio (p-value = 3.7 × 10–12) associated with the top three hashtags demonstrated a significant positive user attitude towards the American Heart Month campaigns. This study underscores the potential of leveraging social media to gain insights into public perceptions of health-related behaviors, enabling researchers and public health practitioners to utilize innovative computational methods for collecting and analyzing large-scale, real-time data from social media campaigns.

摘要
