Breast masses are one of the common symptoms across all ages. Although the majority of breast masses are benign, palpable breast lump is the most common presenting symptom for breast cancer. Evaluation with imaging is indicated in all palpable masses irrespective of the age of presentation. The choice of imaging is primarily based on the age of patient. On both mammography and ultrasound, margin is one of the most important characteristics in assessing if the mass is benign or suspicious. Both contrast-enhanced mammography and MRI provide the functional aspects of the mass through enhancement parameters. Advanced imaging techniques like elastography and diffusion-weighted MRI add as supplementary techniques in further characterising the masses. The majority of the breast masses can be categorised as benign and suspicious based on the imaging findings. Biopsy can be safely avoided for the masses which have typical benign features. Biopsy of the mass is performed to evaluate the masses which are indeterminate on imaging or to confirm the malignancy in case the mass is suspicious. In this chapter we will discuss characterisation of breast masses using basic and advanced imaging techniques and illustrate the imaging approach to breast masses.
