This chapter presents a refined approach to the enhanced-view totally extraperitoneal (eTEP) technique for ventral hernia repair, emphasizing a tailored retromuscular dissection. Recognizing that standard eTEP may be excessive for small to mid-sized defects, the authors propose a modified technique that limits dissection to essential areas, preserving anatomical structures and reducing complications. This method enhances surgical efficiency and patient recovery while maintaining the functional and anatomical advantages of eTEP, including linea alba reconstruction and extraperitoneal mesh placement.
